司法上の手続きや裁判が進む過程でお役立ていただける「治療に関する書類」を発行しております。例えば、心理検査の結果から考えられる心理的特性を考慮し、再犯防止に向けて当オフィスで提供可能な治療計画の提案（治療計画提案書）や、初回の無料相談からの治療経過を全て記述する書面（経過報告書）などです。
書類例：
・治療同意書：治療を受ける側と治療を提供する側、双方の同意を明示
・通所証明書：当オフィスへの通所歴を明示
・治療計画提案書：心理検査の結果を考慮し、当オフィスで提供可能な治療の計画を提案
・経過報告書：初回の無料相談からの治療経過を記述
これら書類を発行する目的は【心理学的な治療を必要とする方々が、出来る限り早く適切な治療へ繋がることを可能にすること】です。多くの場合、反省や我慢、気持ちの強さだけで再犯を防ぐことは困難な場合が多いため、再犯を防ぎ、被害者を生まない状況へと近づくためには、一定期間（一生ではなく）適切な治療・支援を受けていただくことが望ましいと考えております。
拘留中などの場合には、出張による対応も行っております。出張にて面談を行い、事前に実施する心理検査の結果と面談の内容を考慮して目的に合った書類を執筆いたします。福岡にしかオフィスがなく移動の拘束時間と費用が膨大になることを防ぐため、出張費用につきましては最小限に抑えて対応しております。ご相談ください。
また、上記の書類発行後、裁判において書類の内容に関する証言を行うことも可能です。事前の打ち合わせを十分に行い臨みますのですのでご安心いただき、必要な際にはご依頼くださいませ。

The purpose of issuing these documents is to enable people in need of psychological treatment to lead to appropriate treatment as soon as possible. In many cases, it is often difficult to prevent recidivism only by remorse, patience, and strength of feeling, so it is necessary to prevent recidivism and approach a situation that does not cause victims for a certain period of time (not a lifetime). We believe that it is desirable to receive appropriate treatment and support.・Family joint interview: Conducted when the treatment comes back, ends, or when necessary. In case of custody, we also provide a business trip. We will have an interview on a business trip and write a document that suits your purpose, taking into consideration the results of the psychological test conducted in advance and the content of the interview.
